Collection of Personal Health Information
We collect personal health information about you directly from you or from persons acting on your behalf. The personal health information we collect may include, for example, your name, date of birth, address, health history, and information from your assessment. Occasionally, we collect personal health information about you from other sources, if we have obtained your consent to do so or if the law permits.
Uses and Disclosures of Personal Health Information
We take steps to protect your personal health information from theft, loss, and unauthorized access, copying, modification, use, disclosure, and disposal. We take steps to ensure that everyone who performs services for us protects your privacy and only uses your personal health information for the purposes you have consented to.
We use and disclose your personal health information to:
- treat and care for you,
- plan, administer and manage our internal operations,
- conduct quality improvement activities (such as sending client satisfaction surveys),
- compile statistics,
- comply with legal and regulatory requirements, and
- fulfil other purposes permitted or required by law
We will not release personal information to others without your consent, with the following exceptions:
- medical emergencies;
- situations that may involve child abuse;
- situations that may involve danger to public safety, including threats of harm to others;
- situations that may involve danger to your own life, including threats of suicide;
- situations in which your state of intoxication while at the agency or leaving it may give rise to dangers to yourself or others;
- subpoenas from the Court
- other legislation that may require release by TriCAS of personal information it possesses about you
Your Choices
You may:
- access and correct your personal health records;
- withdraw your consent for some of the above uses and disclosures by contacting us (subject to legal exceptions).
